Adult Rehabilitative Mental Health Services
This program is for adults living in Ramsey County who have a diagnosis of a serious mental illness and have difficulty managing their daily activities due to their symptoms of mental illness. Services include:
- Mental illness symptom management
- Mental health service needs
- Vocational functioning
- Educational functioning
- Social functioning, including use of leisure time
- Interpersonal functioning, including relationships with the adult's family
- Self-care and independent living skills
- Medical and dental health service needs
- Obtaining and maintaining financial assistance
- Obtaining and maintaining housing
- Using transportation
Therapies are provided at Lifetrack Resources' main office and other community locations and are available on a sliding fee based on someone's ability to pay. Adult Rehabilitative Mental Health Services are available under Medical Assistance Insurance plans and are provided in the adult's home and other community locations.
