      Certified Occupational Therapy Assistant Pediatric - PT

      Summary: Work in a therapeutic preschool with a dynamic team to provide occupational therapy services to preschoolers from high risk environment.

      Job Duties: Provide occupational therapy services under the supervision of an OTR in the areas of self regulation, fine and gross motor development, visual motor development and self care training. Responsibilies will include treatment and documentation, maintaining a clean and safe work environment, attending team meetings and assisting the OTR in program development and inservicing.

      Qualifications: Associate degree from an accredited two-year program.

      Requirements: Prefer minimum of one year clinical experience.

      Employment Counselor / Advancement Specialist - FT

      Summary: Provide intensive employment counseling and job development services to participants (immigrants and refugees) in the CAREER program. Work with employers to develop partnerships in order to place participants in employment. Meet quality standards and goals to ensure continuation of grant funding.

      Job Duties: Conduct intake with potential participants; assess the skills and interests of participants and set realistic job goals. Meet with participants on a regular basis to assist them in becoming self-sufficient in the job search process. Use other assessment tools and counseling strategies to uncover and remove barriers to employment. Refer participants to skills-enhancing activities and prepare participants for job interviews. Provide assistance with small group trainings on job-seeking skills.

      Establish and maintain mutually beneficial partnerships with companies to place qualified participants in openings. Locate job openings using a variety of methods. Work cooperatively with collaborative partners, share job leads and needs to fill positions with most qualified candidates. Meet contract goals for placement and other performance objectives.

      Develop job advancement opportunities for participants in order to assist participants in advancing into competitive employment. Follow up with employers and participants after placements to promote job retention. Schedule job-coaching with employer and job coach as appropriate to increase job retention.

      Develop case file on each participant, case note all participant activities and maintain accurate records of all correspondence. Maintain accurate records in program databases and complete all internal and external paperwork in a timely manner. Make appropriate referrals to community resources. Perform other tasks as needed to meet Lifetrack and unit objectives.

      Qualifications: Bachelors in social services, preferred.

      Requirements: 1 year experience with employment issues. Knowledge of job seeking skills. Experience training individuals and small groups. Experience working with refugees or diverse populations. Knowledge of MFIP helpful. Bilingual helpful.

      MFIP Family Stabilization Services Counselor - FT

      Summary: Assist families who are not making significant progress in MFIP due to a variety of barriers to employment. Utilize a strengths based method to assist participants in overcoming barriers.

      Job Duties: Manage a caseload of approximately 60 participants. Provide services that will help participants and their families: obtain and retain employment, improve family stability, increase economic stability and reduce barriers. Identify family needs by thorough assessment of physical and mental health, social functioning, vocational, and financial situation. Assist client in establishing appropriate goals to address barriers and find employment. In consultation with client, develop an employment plan, estimating the length of time it will take for the client to accomplish goals in the plan. Case note, and report client progress as required by the particular program involved. Initiate and coordinate referrals to resources that can assist in addressing family needs. Keep abreast of factors or programs which may affect client services. Meet program goals. Keep Lifetrack management informed of critical issues or factors which may impact the organization. Continuously strive for self-improvement. Perform other duties as assigned.

      Qualifications: Associates Degree and two years equivalent experience, prefer Bachelors degere in social service or related field.

      Requirements: Strong oral and written communication skills, strong planning and organizational skills, ability to work effectively with clients of diverse backgrounds as well as with government agencies, employers, and other staff members. Demonstrate professionalism with honesty and integrity in all business actions and communications. Strong attention to detail while effectively delivering services to clients.       MFIP Job Counselor - FT

      Summary: Provide intensive case management and job placement assistance services for Minnesota Family Investment Program participants seeking durable employment. Utilize a strengths based method to assist participants in addressing and overcoming barriers.

      Job Duties: Maintain a caseload of approximately 60 participants. Authorize childcare, issue and track supportive services, facilitate group meetings, case note, and maintain files to adhere with DHS (Department of Human Services) guidelines. Direct participants in the job search process by providing job seeking skills, and encouraging attendance in job clubs and workshops.

      Qualifications: Associates Degree and two years equivalent experience, prefer Bachelors degree in social service or related field.

      Requirements: Social service or previous MFIP experience preferred. Must have excellent customer service and time management skills. Needs to work well with diverse populations and be sensitive to confidential information.

      Physical Therapist-Pediatrics - PT

      Speech / Langauge Pathologist - On-Call

      Summary: Seeking qualified speech pathologists interested in on call / weekend coverage of adult clients in an acute hospital setting. Hospitals are located in St. Paul and eastern suburbs. This position may have the potential for regular hours.

      Position Duties: Provide evaluation and treatment to adult clients for a variety of diagnoses, including aphasia, dysarthria, cognition, and swallowing. Evaluations will include clinical speech/language and cognitive evaluation, bedside swallow assessment, and video fluoroscopic swallow studies. Must be independent with dysphagia evaluations and treatment. A qualified therapist will be effective, not only in providing speech therapy services, but in time management, completion of the necessary documentation, and communication with other professionals.

      Qualifications: Must hold a certificate of clinical competence in speech pathology and be eligible for MN licensure in speech pathology.

      Requirements: Experience with adult clients preferred.

      Hours Per Week: On call / occasional weekend days with the potential for possible growth into regular hours.

      Speech / Langauge Pathologist (Pediatrics) - FT School Year

      Summary: Provide direct client services to children, including evaluation, treatment planning and implementation, ongoing communication with client, family, team members and others as appropriate, along with discharge planning. Majority of direct service at charter schools within the Twin Cities Metro. Provide services to clinic clients as schedule allows.

      Position Duties:
      1. Provide speech services to charter school clients and clinic center clients.

      2. Maintain compliance with state special education requirements for assessment, direct and indirect service, consultation, documentation and meetings.
      3. Complete in center evaluations, select, plan and coordinate speech and language services, provide consultation to family and school, maintain frequent commnication with other disciplines involved with client, maintain case records, and implement discharge plan.
      4. Maintain and upgrade clinical skills and participate in program and agency meetings as appropriate.

      Qualifications: Masters degree in speech pathology from a university or college program accredited in speech pathology from the American Speech-Language-Hearing Association (ASHA), or a university or college program whose Speech Pathology Chairperson or Supervisor is ASHA certified in speech pathology.

      Requirements: Must be lincensed in the State of Minnesota to provide speech therapy services. Must have a Minnesota Department of Education license.

      Minimum of 260 hours of supervised practicum experience in speech pathology from the training institution granting the Masters degree. Related job experience preferred.
